7 A document which outlines the care needs and interventions for a person. This may also be known as a support plan. Care plan: Providing personal care to people of all ages. Helping people who require support as a result of a disability or health concerns, care workers provide support services to assist people with daily living. These are paid positions and may also be referred to as Direct Care Workers or Support Staff. Care Worker: A written record used by workers and others involved in care. This may be an in-home file, electronic system or hand written case notes. Co-ordinators are usually responsible for ensuring important information is disseminated to those involved in care such as other services, health professionals and any family or significant others. Client Journal: Clinical care is health care that encompasses the prevention, treatment and management of illness or injury, as well as the maintenance of psychosocial, cultural, spiritual and physical wellbeing. Clinical Care: The person who is receiving help. Other terms include client, care recipient, patient, and resident. Consumer: A paid person who provides support, leadership and direction to care workers. They also are known as Case Managers, Case Workers, Care Supervisors, Support Advisors, Lifestyle Facilitators.
