25 Medication Support Safe oral medication support 1 2 3 4 Know your client: • Understand relevant medical condition related to their medication • Check care plan for instructions regarding storage / handling Before the task: • Wash hands and apply gloves and assemble appropriate equipment i.e. Dose Administration Aids, glass of water, saucer • Check chart for special instructions • Identify the client and explain the procedure to the client before assisting During the task: • Check the identity of the client and check the medication against the Medication Plan. Remember 5 R’s PRNTD Right Person Right Route Right Number Right Time Right Documentation • Assist client to remove tablets from dose administration aid (if able) • Provide client with water to assist in swallowing medication • Observe the medication is swallowed After the task: • Remove gloves and wash hands (it is not mandatory to wear gloves unless you are handling the medications) PPE is always recommended for infection control • Sign signing sheet • Note any adverse events in the Client Journal and complete a Medication Incident Report, if required Document in the Client Journal: • Enter note into the Client Journal to notify that medication was taken – note should also be entered to notify if / when medication was refused 5 Guide for consumers, carers and workers
