37 Observations Supervising blood glucose readings 1 2 3 4 Know your client’s diabetes conditions: • Check care plan for alerts Before the task: • Assist client to prepare equipment (meter, lancet devise with lancets & blood glucose strips) • Check expiry date on strips and they are correct for the meter • Check meter shows calibration correctly • Check with client which fingers used and ensure rotation • Attend hand hygiene and apply PPE (gloves) if undertaking the task During the task: • Prick tip of finger with the lancet • Add small drop of blood onto a strip ensuring fully covered • Insert strip into meter • Record time and reading into diary / documentation After the task: • Wipe lanced area with tissue • Place cap back on strip container • Turn off meter • Discard strip and PPE Document in the Client Journal: • Change in food or drinking habits • Reports of dizziness, feelings of being unwell 5 Guide for consumers, carers and workers
