47 1 2 3 4 Know your client’s abilities: • Check care plan for alerts and manual handling instructions • Understand client’s ability to carry out instructions • Understand client’s restrictions while maximising function Before the task: • Check manual handling instructions and equipment (e.g. shower chair, toilet raiser) • Prepare the bathroom • Engage with client while assisting them to choose clothing and prepare bathing needs • Check with male clients if shaving is preferred in the shower • Offer the client use of the toilet before the shower procedure • Encourage perineal hygiene followed by hand hygiene (prompt toilet paper use front to rear to reduce risk of infections) During the task: • Attend hand hygiene • Check water temperature first (turn cold water on first then adjust with warm water) • Assist transfer to seat/shower chair and assist with undressing as per abilities • Maintain dignity by using cape / towel as a screen as preferred • Promote independence in a rush-free environment encouraging stretching • Provide bathing cloth to client for use while assisting with difficult to reach areas • Note skin condition (in discreet manner) After the task: • Assist with drying: ensure skin folds and between toes are dry • Assist with dressing as required Document in Client Journal: • Skin changes (redness, bruised or broken skin) 5 Showering and toileting
