65 Flow Chart for Clinical Care The following steps are recomended for clinical care interventions: • referral received for clinical care or consumer identified as requiring clinical care • assessment of need using validated tool from assessment tool suite • best practice advice sought from others if required • document results of assessments with determined clinical intervention • detailed clinical intervention to be provided recorded in care plan • consumer participation documented in care plan • detailed work instructions for care staff provided in care plan • specific care plan may be required for complex care e.g. wound care, medication, stoma care, dementia strategies, swallowing difficulties • health information sheet provided to other staff involved in care • health information sheet provided to consumer / others • highlighted important information to be observed and reported • summary of actions entered in progress notes • documented, dated evaluation of interventions
