HOME CARE PACKAGE (HCP) Individualised package of care West Coast Homecare was approved as an Age Care Provider on 1 June 2017 under section 8-5 of the Aged Care Act 1997 (the act). The Home Care Package is delivered based on what the individual requires so they, or their family/ or carer can select the services they want, delivered by West Coast Homecare as their provider of your choice. The three main categories of services are: 1. Services to keep people well and independent – including personal care, nursing services, allied health. 2. Services to keep people safe in your home – including cleaning, home maintenance and modifications, assistive technology. 3. Services to keep people connected to your community – including transport, social support services. 4. The package level assigned is based on individual consumer needs.
