What is ISBAR? ISBAR originated in the US Navy and is a standardised format that assists the transfer of information particularly when there is a time limit or a need to communicate succinctly. Its effectiveness has been demonstrated in clinical and non- clinical situations. When communicating verbally it ensures conversations are clear and focused which can assist in improving consumer / patient safety and reducing risk of error in care provision. What is great is that it is a simple, portable and memorable tool. We hope you find it useful. Identify Name of person reporting and role, name of person being discussed Situation What is the main problem at present, details of the event or current situation Background What may have led to situation, person’s medical history, initial treatment Assessment / Actions: Injuries, observations, cognition, is this normal for person, what has been done – collate all the information.
