Continuous improvement folders
Completing FM Bus-16.1 Improvement Logs or FM Bus-16.2 Confidential Improvement Logs are your
way to identify and communicate:
•
Issues/concerns
•
Suggestions for improvements
•
Positive feedback
These forms are contained in the Continuous Improvement Folders in each care home.
Hazard reporting
If you identify a hazard it is your responsibility to report it immediately, both:
•
Verbally to your GM/RN/person in charge and
•
In writing by completing a Improvement Log which you will find in the Continuous Improvement
folder
You may also need to post signage to prevent others being exposed to the hazard.
Incident reporting
If a resident or a visitor has an incident or an accident in the care home it is your responsibility to
report it immediately both:
•
Verbally to your GM/RN/person in charge and
•
In writing by completing a FM Res-4.24 Resident Incident Notification or FM PM-10.6 Visitor
Incident Accident Notification
You must complete these even if no injury has occurred as it may help identify the risk of injury to
others.
Repair reporting
If you find faulty or broken equipment, fittings or items requiring repair it is your responsibility to
report it:
•
Verbally to your GM/RN/person in charge and
•
In writing by completing a FM CHS-04.1 Maintenance Log to communicate the required repair
You can also report unscheduled cleaning issues via a FM CHS - 03.1 Cleaning log.
